Is Physio Covered by OHIP? Insurance, Direct Billing & What to Know

If you’ve been told you need physiotherapy, one of the first questions that probably comes to mind is: how much is this going to cost me? It’s a completely fair question, and the answer depends on a few things — including the type of insurance you have. Is Physiotherapy Covered by OHIP?

This is one of the most common questions we hear, so let’s get straight to the point: in most cases, physiotherapy is not covered by OHIP (Ontario Health Insurance Plan) for the general public.

There are a few exceptions. OHIP does cover physiotherapy services for eligible Ontarians in specific circumstances, including:

  • People aged 65 and older
  • Children and youth under 19
  • People receiving Ontario Disability Support Program (ODSP) benefits
  • Patients discharged from hospital after a recent surgery or acute illness

If you fall into one of these categories, you may be able to access physiotherapy at an OHIP-funded clinic at no cost — but spaces can be limited. For everyone else, coverage typically comes through private or employer-provided insurance plans.

Extended Health Benefits: Your Most Likely Source of Coverage

Most working Ontarians have access to some form of extended health benefits through their employer, union, or a private plan they’ve purchased. These plans often include a physiotherapy benefit — though the amount and conditions vary quite a bit from plan to plan.

What Does Extended Coverage Typically Look Like?

Extended health plans usually cover a set dollar amount per year for physiotherapy — anywhere from $300 to over $1,000, depending on your policy. Some plans require a referral from a doctor; others don’t. Many also have a per-visit cap or require you to meet a deductible first.

Here’s what we’d suggest doing before your first visit:

  1. Log into your benefits portal or call your insurance provider directly
  2. Ask how much you have available for physiotherapy this benefit year
  3. Ask whether a physician’s referral is required
  4. Confirm whether direct billing is available

A few minutes of prep can save you from unexpected out-of-pocket costs later on.

What Is Direct Billing and How Does It Work?

Direct billing means that instead of you paying upfront and submitting a claim yourself, your clinic bills your insurance company directly on your behalf. It’s a convenience that many patients really appreciate — especially when you’re already dealing with pain or injury and don’t want to manage paperwork on top of it.

What If You Don’t Have Insurance?

Plenty of people pay for physiotherapy out of pocket — and many find it well worth it. Physiotherapy has strong research support for a wide range of conditions, from sports injuries and post-surgical recovery to chronic pain management. A study published in the Journal of Orthopaedic & Sports Physical Therapy found that early physiotherapy intervention was associated with significantly reduced downstream healthcare costs — meaning getting treatment early can actually save money in the long run.

If budget is a concern, talk to your physiotherapist about a realistic treatment schedule. Many conditions can be addressed effectively with a focused number of visits, especially when paired with a home exercise program.

Motor Vehicle Accidents and WSIB Claims

If your injury was the result of a car accident or happened at work, different funding sources may apply:

  • Motor Vehicle Accident (MVA): Physiotherapy is generally covered under Ontario’s standard auto insurance policy through your accident benefits coverage.
  • WSIB (Workplace Safety and Insurance Board): If your injury is work-related, WSIB may cover your physiotherapy at approved providers.
